In brief

A Health Insurance Underwriter (also called a Medical Underwriting or Claims Specialist) reviews medical records, policy wording, treatment details and risk factors to decide whether to approve a health-insurance policy or claim, and on what terms. In India, the usual entry is a graduate degree — often in life sciences, nursing, pharmacy, BDS/MBBS, or a commerce/management background — plus insurance process training. A certification from the Insurance Institute of India (III) can help. Starting pay is about ₹3-7 lakh a year.

What does this job really involve?

Underwriters and claims specialists are the careful readers of the insurance world. In underwriting, you assess a customer's medical and lifestyle information to decide if a policy can be issued and at what premium. In claims, you check hospital documents and policy terms to approve, query or decline a claim fairly.

Picture this: a hospital claim comes in for a surgery. The specialist checks the discharge summary, bills, policy wording and waiting periods, confirms everything is genuine and covered, and approves the correct amount — protecting both the customer and the insurer.

Would you enjoy this work?

It may suit you if…

  • You are careful, detail-focused and comfortable reading medical documents.
  • You can weigh risk and make consistent, rule-based decisions.
  • You have (or want to build) a background in health, life sciences or insurance.
  • You are good with policy wording, records and basic spreadsheets.
  • You prefer analytical, desk-based work over field or sales roles.
💰

Pay, demand & where it's heading

Health insurance is one of India's fastest-growing financial sectors, so insurers, third-party administrators (TPAs) and insurtech firms need people who can assess risk and process claims accurately. A medical background is a real advantage for underwriting and claims roles.

  • Approve, query or decline, and record clear reasons.
  • Coordinate with doctors, hospitals, TPAs and sales teams.
🎓

What you'll need to get in

  • A graduate degree — life sciences, nursing, pharmacy, BDS/MBBS, or a commerce/management background.
  • Insurance process training; an Insurance Institute of India (III) certification helps.
  • Good grasp of medical terminology and health-insurance basics.
  • Careful, analytical reading and consistent decision-making.
  • Comfort with records, policy wording and spreadsheets.
🧭

Ways to get in — the study paths

There's more than one road. Pick the one that fits you:

1. Trainee role in an insurer, TPA or insurtech · most common route
graduate degree (medical / life-science / commerce) claims or underwriting trainee role on-the-job training Underwriter / Claims Specialist
The usual direct entry, especially for medical and life-science graduates.
2. Add an insurance certification · skill boost
graduate degree Insurance Institute of India (III) or health-insurance / claims certification underwriting / claims role
Builds formal policy and claims knowledge that employers value.
3. PG Diploma in Insurance or Risk Management · deeper route
graduate degree PG Diploma in Insurance / Risk Management underwriting / insurance-operations role
Helpful for stronger underwriting and risk roles.
4. MBA / PGDM for management growth · optional
graduate degree MBA / PGDM (Insurance, Finance or Healthcare Management) manager track
Optional; supports faster growth into management roles.
Good to know: a medical or life-science background is a strong advantage for underwriting and claims, but commerce and management graduates also enter through claims-operations roles. An III certification is useful but is generally a skill bridge, not a replacement for employer-specific training. There is no single compulsory licence for these roles — always check what the employer asks for.

Common questions

Do I need a medical degree?+
Not always. A medical or life-science background (BDS, MBBS, B.Pharm, Nursing, B.Sc) is a strong advantage for medical underwriting and claims, but commerce and management graduates also enter through claims-operations roles.
What certification helps?+
A certification from the Insurance Institute of India (III), or a health-insurance / claims-management course, builds useful policy and process knowledge. It is a skill bridge, not a compulsory licence.
Is underwriting the same as claims?+
They are related but different. Underwriting decides whether to issue a policy and at what premium; claims decides whether a submitted claim is approved. Many people work across both over time.
Is this a desk job?+
Mostly yes. It is analytical, document-based work, unlike field or sales roles in insurance.
What is the starting pay?+
Usually around ₹3-7 lakh a year, rising to about ₹10-20 lakh with experience and seniority. A guide, not a promise.
